Welcome to the Critical Incident Reporting (CIR) Form for Housing Providers

This form is designed for housing providers to report critical incidents that occur within BC Housing-funded buildings or programs. Reporting these incidents helps ensure the safety, well-being, and protection of tenants, staff, and the broader community.

When to Submit a Critical Incident Report 

A Critical Incident Report (CIR) must be completed and submitted to BC Housing as soon as possible following any serious incident.

What is a Critical Incident

A critical incident is defined as an incident that is significant enough to threaten the continuous operation of the services of a service provider.
Examples of serious incidents include, but are not limited to:

  • Fire, flood, or other major property damage

  • Death (suspected overdose)

  • Death of clients or staff on site (not overdose related)

  • Infectious disease outbreaks

  • Serious violence or injury

  • Police, Fire, or Ambulance response involvement

  • Media or reputation-sensitive events

  • Any situation requiring urgent BC Housing notification or follow-up

Reports must be submitted with 12 hours of an incident being identified.
